A fishing boat is sailing the boundless ocean. The passengers are quiet, and don't say a word to one another. They can't wait for the boat to reach its destination-Shikina Island.
Among the passengers is a young man-his face, pale from being seasick. Takehiro, a young boy also on board, is feeling nauseous himself. He looks at the young man, and in an attempt to comfort him, he crosses his finger and says, "This is a charm that prevents you from getting seasick." The young man returns a smile. Takehiro asks the young man his name.
"Kensuke Goto..." answers the man, in a feeble voice.
As Kensuke is speaking, the sound of the roaring engine causes Takehiro to think that Kensuke says his name is "... Coto."
Four hours later, Kensuke Goto and the others finally reach the small island. Kensuke has volunteered to work there because it lacked a medical doctor. Shoichi Hoshino who accompanied Kensuke on the boat, works at the town hall in the Public Welfare Division. For a long time, Shoichi had painstakingly searched for a doctor that was willing to work on the island. So he truly welcomes Kensuke's arrival with open arms. However, Taketoshi Hara, the owner of the boat and the other islanders don't seem to have much interest in their new doctor. They are confident that Kensuke will grow tired of working there, and leave the island sooner or later.
Upon his arrival, Kensuke is taken directly to a man's house. What he finds there, is the dead body of an old man. His family is in need of a death certificate. Without a death certificate, they are unable to give the deceased a funeral. Before Kensuke arrived, they would have had to transport the body six hours by boat until they reach the mainland in order to obtain a death certificate. Sensing what Kensuke's thinking, someone says "You're disappointed at this place, aren't you?" Hearing this, Kensuke immediately turns to Ayaka, Shoichi's daughter who works as a nurse at the clinic.
The so-called "clinic," is a small building with very few medical instruments. And the only person present to welcome Kensuke is Kazunori Wada, the clerk.
With what little equipment he could find, Kensuke does his best to prepare for the patients who are about to arrive.
In the eyes of Tsuruko Uchi, the "living fossil" of the island and Shigeo Ando, a fisherman, Kensuke seems like a helpless young guy. He doesn't even have a driver's license to get himself around the island.
One day, Taketoshi's only son Takehiro, is carried into Mariko Nishiyama's restaurant. Takehiro not only has acute appendicitis, but also peritonitis, and needs to have surgery right away. But Taketoshi will not allow Kensuke to operate on his son, and insists on taking him to a hospital on the mainland. He prepares his boat, and is about to depart. Kensuke is confident that this will only make matters worse and that Takehiro's life is in danger. So he jumps onto Taketoshi's boat along with Ayaka and Kazunori.
"Shikina Island"
The Fuji Television drama, "Dr. Coto's Clinic" takes place on the fictitious Shikina Island. The drama was actually filmed on location on the beautiful Yonaguni--one of the islands in the Yaeyama island chain.
Yonaguni Island is located some 2,030 km from Tokyo, 509 km from Okinawa, and 111 km from Taiwan. On a sunny day, the mountain range of Taiwan is visible from the island. The sea is filled with coral reefs, and is a very popular diving spot. The sea surrounding Yonaguni Island is the home to the dogtooth tuna, and hammerhead sharks. The ancient ruins located underwater, are the subject of intense interest of archeologists today. Yonaguni is also known for its fishing spots, where marlin weighing 80-200 kg, are regularly reeled in. Some lucky fishers have even come across marlin larger than 400 kg.
The Yonaguni Horse is the smallest of the eight different types of native Japanese horses that exist. Another unique creature of Yonaguni Island is the world's largest moth called yonagunisan (or ayami habiru.) Its wing span can exceed 30 cm.

I like the way the set the doctor up on the isolated island having to help people that really didn't trust modern medicine. They had some very good plots for the different episodes, like when the kid got sick and the doctor had to do the surgery. But, the young guy that played the doctor was pretty pathetic. His reaction to every obstacle was to almost threw up. His character in real life would never get a patient anywhere on earth. I would rather do the surgery on myself and take my chances. Dr. Koto is a kind hearted doctor who's goodness supercedes his ambitions. In a day and age when most of the young adults move on to the city or larger towns, small village doctors are needed especially in remote islands. The main character is a well known actor famous from Kita no Kuni Kara and he looks the same. Ｉ was recommended this drama and what a surprise it turned out to be. This was one of the highest rated dramas of 2003, and there's plenty of reason to support that. It's a touching drama about a doctor and the people on the island. It really touches on the subject of human nature, friendship, bonds, and trust. Ａｔ ｆｉｒｓｔ Ｉ thought it was going to be a slow and dull drama, but after watching the first 10 minutes of it, I totally got into the whole set up of things. The characters are so powerful and the music is great. I love watching dramas that you can really get yourself into, and this is definitely one of those.

I thought this was an excellent drama indeed. Not many mainstream stars in this show except Shibasaki Kou but there many pretty ladies anyway :P
This drama tells the tale of Dr Koto coming to an island to work only to find that the entire community doesn't trust him. He slowly but surely wins them over through his dedication and sincerity to the medical profession, as well as his affiable character which allows to relate to the people with ease. This has really nice drama theme based on compassion for others and understanding the true meaning of being a doctor. A light hearted drama overall, but gets a bit serious toward the end.

I now consider this the best drama I've seen ever! No matter how many times I see it, I always get teary-eyed and touched by the good-naturedness of the characters. Call me old-fashioned but I find nothing wrong with this kind of worldview. Though, I doubt if anyone's as noble as Dr. Koto (he's just too kind and pure!)
As for the actors, I just love Yoshioka Hidetaka! He's amazing! I discovered this drama while looking for Sakai Masato dramas (only to find out later that he only showed up in the 2nd season). But with just the first ep. of Dr. Koto, i fell in love with him (Dr. Koto) in an instant! Some actors were just as convincing like Izumiya Shigeru (Shige), Sengoku Noriko (Uchi), and of course, Kakei Toshio (Wada). Even Tomioka Ryo (Takehiro) did amazingly well!
And who can forget about the OST! It's just perfect. I love the instrumental versions of Gin no Ryuu no Sei ni Notte and Omoide Dake dewa Tsura Sugiru. One of the best drama OSTs I've heard.
This drama isn't getting than much attention b/c the actors here are not the usual "idols", except maybe for Shibasaki Kou. And the story isn't the usual "love story" either. But I'm glad it was well received in Japan. | | Reviewed by moeru_h on 3 September 2008 |

Very good location The nurses are good. The Doctor is a joke. It is ruined by the main character. Just finished watching this drama recently...thought it would be just like any other doctor dramas I've watched before..but I was wrong...first..the scenery are really beautiful..the actors and actresses are all really talented (especially those playing parts of the villagers) and the kids are soo talented too...a very nice drama indeed. It's too slow. There's just something about it that doesn't make me care about any of the characters. The doctor's character is so soft that I wanted to slap him around sometimes and say PULL IT TOGETHER, MAN! The only lively thing in the show was the old woman and the pissed off nurse. Dr. Koto's clinic is definitely one of the best series of all time. It impresses not only in the accuracy and realism of surgical procedures, but also in it's heart-warming stories. From the very beginning it was clear that there would a lot of smiles and a lot of tears. The entire case was amazing and the staff did a fantastic job.
