It might be just like when Reita hurt his finger during the NLSG tour. During the show they are so hyped up that they dont feel the pain so they do it.

Yeah I have to agree with this. There are many cases when pain is numb because a person is hyped up. Maybe the neck gives him hell later but at the time it doesnt seem to matter especially to him. Then again, by now i think all of us have come to realize that the GazettE do just about anything to put on a show they can enjoy and the fans can enjoy too. Somehow I dont see it in Ruki's character to just stop headbanging just because the guy thinks its bad for him.

i've been through heartsurgey twice~ was born with a partly dysfunctional aortha whatever it is called in english thing. kinda important thing. you know the door.. that ehm... opens and closes to push the blood forward.
so i've been in hospital XD

For me it would depend where they play:

Paris, France: 3 and a half hours by plane
London, England: 4 and a half hours roughly, by plane
Germany: not too sure, but about 5-6 hours by plane
Finland: No clue but further than the first two definitely

Well if i recall correctly it was only the two of them there, so unless they did a two man show they didnt have a show.
